Your right to know what care will cost
If you do not have insurance, or you have it but choose not to use it, federal law gives you the right to a written estimate before services begin. No surprises later.
What a Good Faith Estimate is
It is a written breakdown of what your care is expected to cost. It lists the services we plan to provide, how many sessions or hours we expect, and the price of each item, so you can see the whole picture before you commit to anything.
It applies to people who are uninsured, and to people who have insurance but decide to pay for their care themselves rather than file a claim.
When you get one
- Whenever you ask for one. Ask at any point, before or after scheduling, and we will put it in writing.
- Automatically when you schedule. If your appointment is at least three business days away, we give it to you within one business day of scheduling. If it is at least ten business days away, we give it to you within three business days.
- In writing, on paper or electronically, whichever you prefer, and in English or in Spanish.
For ongoing care such as weekly counseling or an ABA program, the estimate covers the period we can reasonably project, and we issue an updated one when the plan changes.
If the bill comes in much higher
If you receive a bill that is at least 400 dollars more than your Good Faith Estimate for any single provider, you can dispute it. You have 120 calendar days from the date on the bill to start the federal dispute resolution process.
Starting a dispute does not put your care at risk and we will not retaliate. Keep a copy of your estimate, it is what the process compares the bill against.

How to ask us for yours
Call the office at 305-602-0268 or email info@denalibehaviorcrt.com and say you want a Good Faith Estimate. Tell us which service you are asking about, ABA or counseling, and we prepare it. There is no charge for the estimate itself.
If you do have insurance and plan to use it, this notice is not the document you need. Our team will verify your benefits and explain your copay, deductible and any authorization your plan requires before services start.
